Public website versus patient portal
This public website provides general information about Clarity Psychiatry, PLLC. Visiting the website does not create a provider-patient relationship. Do not use public website fields or ordinary email to send symptoms, diagnoses, medication information, medical records, or other sensitive health information.
Information processed when you visit
Website hosting and security providers may process technical information needed to deliver and protect the site, such as IP address, browser/device information, referring pages, timestamps, and basic request logs. If analytics, advertising, or other optional tracking tools are enabled in the future, the practice will configure available privacy controls and update this notice as appropriate.
Embedded scheduling and portal services
The appointment calendar or patient-portal links may be provided by CharmHealth or another designated clinical technology vendor. Information you enter into an embedded clinical widget or secure portal is handled through that clinical service rather than an ordinary Clarity Psychiatry website form. The vendor’s own privacy and security terms may also apply.
Clinical privacy
Protected health information maintained by Clarity Psychiatry is governed by the practice’s HIPAA Notice of Privacy Practices, applicable federal law, and applicable Arizona law—not only by this website privacy statement.
Email and ordinary text messaging
Unless a communication channel is specifically identified by the practice as an approved secure method for clinical communication, do not assume ordinary email or SMS is appropriate for protected health information. Use the secure patient portal for clinical messages.

This custom site does not require advertising cookies to function.
