No provider-patient relationship from website use

Viewing the website, clicking a link, requesting an appointment, or sending a nonclinical inquiry does not by itself establish a provider-patient relationship. A clinical relationship begins only after required intake, eligibility, consent, and other practice procedures are completed and the practice accepts the patient for care.

Telehealth eligibility and location

Clinical services are intended for adults who are physically located in Arizona at the time care is rendered, unless Clarity Psychiatry later confirms lawful authorization to practice in another jurisdiction. Telehealth may not be clinically appropriate for every person or condition.

Emergencies

This website, scheduling system, patient portal, and routine messaging are not emergency services. In an immediate medical or psychiatric emergency, call 911 or go to the nearest emergency department. In the United States, 988 is available by call or text for crisis support.

Medication and controlled-substance disclaimer

No prescription or particular medication is promised by scheduling or completing an appointment. Prescribing decisions depend on diagnosis, clinical indication, risks and benefits, available records, identity and location verification, monitoring requirements, clinician judgment, and current federal and Arizona law. An in-person examination or other assessment may be required.

Third-party services

Scheduling, telehealth, payment, patient-portal, maps, analytics, or other features may be provided by third parties. Their terms and privacy practices may apply to your use of those services. Clarity Psychiatry is not responsible for third-party websites outside its reasonable control.

Accuracy and changes

The practice aims to keep website information accurate, but services, fees, availability, laws, clinical standards, and technology may change. Information in the secure portal or documents provided directly to an established patient controls if it conflicts with general website content.
